Insurance is ending coverage for some key drugs that affect the health of patients. It’s a David-vs.-Goliath dispute, and millions of patients are caught in the middle — perhaps even unaware they’re about to lose coverage for compounded medications they need for their conditions. At issue are the customized medications pharmacists prepare for patients who can’t metabolize or tolerate commercial drugs. Compounded medicines often are the only option for doctors treating certain children and seniors, patients coping with the pain of cancer and diabetes, and those with liver or kidney diseases.
In one corner: powerful insurance companies and pharmacy benefit managers (PBMs) hell-bent on saving money by eliminating or cutting coverage of those medications.
In the other corner, fighting to reverse their moves: a coalition of patients, physicians, pharmacists and pro-patient groups like the Veterans Advocacy Group of America, the Kidney Cancer Association and the Arthritis Foundation.
“This is about shifting costs to patients,” says Jay McEniry, executive director of Patients and Physicians for Rx Access (saverxaccess.org). “Physicians are being placed in the impossible position of either prescribing a compounded medication the patient needs but can’t afford, or prescribing a less effective treatment because it may be covered by the patient’s insurance.”
The list of “Goliaths” who’ve announced or already implemented such cutbacks now includes United Healthcare/Optum Rx, Catamaran, CVS/Caremark, Harvard Pilgrim and Blue Cross Blue Shield plans in several states.
But the coalition’s immediate wrath is directed at the nation’s largest PBM: Express Scripts, which in September is slated to stop covering 1,000 drug ingredients commonly found in compounded medications — effectively “eliminating an entire class of medications,” says McEniry.
Express Scripts and others argue that commercial drugs can do the job just as well for less money. But try telling that to patients like Linda Sauer.
The Dwight, Illinois, woman relies on her doctor-prescribed compounded medications for relief from several painful and debilitating conditions, and is outraged that Express Scripts’ decision leaves her no choice but to pay for them out of pocket.
“They’re denying me access to medicines that work better than the mass-produced drugs I’ve tried,” she says. “It will cost me and others hundreds of dollars per month.”
Sauer at least has read the advisory notice from Express Scripts, which the coalition claims gives “misleading reasons” for targeting what it calls “essential medicines” whose ingredients are purchased from FDA-regulated suppliers. But what of patients who didn’t?
Sadly, they’re in for a shock the next time they try to fill a prescription.

Using one of these bags doesn’t guarantee that you can leave your laptop in your bag for screening. If a Transportation Security Officer finds that the bag does not present a clear and distinct image of the laptop separate from the rest of the bag, the laptop must be screened separately.

Age-related macular degeneration (AMD) is a disease of the retina. End-Stage AMD is the most advanced form of the disease and the leading cause of irreversible central vision loss and legal blindness in individuals over the age of 60. This blind spot is different than the visual disturbances experienced with cataracts (clouding of the eye’s lens) and is not correctable by cataract surgery or eyeglasses. Side vision, or peripheral vision, is not affected by AMD, but is too low resolution to make up for lost central vision. At this time, there is no cure for End-Stage AMD and no way to reverse its effects, but there is a novel treatment available for qualifying candidates. What is CentraSight® and the Telescope Implant?
FDA approved, the pea-sized telescope implant is intended to improve distance and near vision in people who have lost central vision in both eyes because of End-Stage AMD. The telescope implant is surgically placed inside one eye. The implanted eye provides central vision; the other eye provides peripheral vision.
The telescope implant is not a cure for End-Stage AMD. It will not restore your vision to the level it was before you had AMD, and it will not completely correct your vision loss. Patients with this level of AMD have had to cease driving due to their vision; after the telescope procedure, although near and distance vision may improve, driving will not be possible because the implant does not restore normal vision. Are you or a loved one a candidate?
In general, to be considered a potential candidate for the telescope implant an ophthalmologist must first confirm that you:
Have irreversible, End-Stage AMD resulting from either dry or wet AMD
Are no longer a candidate for drug treatment of your AMD
Have not had cataract surgery in the eye in which the telescope will be implanted
Meet age, vision, and cornea health requirements
After the ophthalmologist confirms that you are a potential candidate, your vision will be tested using an external telescope simulator. The results of the test and visual training/rehabilitation evaluation visits will help you and your ophthalmologist decide if you are likely to benefit from the CentraSight treatment program, which includes an out-patient surgery followed by low vision rehabilitation.
The telescope implant is not a cure that “sees” for you. For the telescope implant to work, patients need to work with low vision specialists as well as practice with the device at home. Realistic Goals:

Various hobbies like painting, knitting or gardening
Unrealistic Goals:
Driving
Seeing a golf ball in flight
Playing tennis
Never having to use a magnifying glass again

So does that mean that you’re going to spend hundreds of extra dollars on a set of clubs with a custom fit? While custom clubs are one answer, most people can find what they need from a well-stocked golf store. But there are some things to keep in mind.
Length of the club is important, but it’s not everything. The pros say that the size of your golf club’s grip should be comfortable for your hands. That’s why clubs for younger players and women often have a smaller grip.
If you should decide to go for a set of custom clubs, what is the process? It’s nothing like be measured for a suit. The point is not only to make the club reach from the point of your outstretched hand to the ground, but also to make the most of your body – your strengths and your weaknesses. So don’t go for your custom golf club fitting session expecting to spread your arms and stand still while someone uses a tape measure.
A good custom fit will probably take place both inside and outside. The best custom fit clubs will be tailored so that your swing is taken into account. Sound expensive? Possibly.
While some major golf club manufacturers will charge (dearly) for the actual “fitting,” many offer up this service for free, with your commitment to buy clubs from that company. While custom clubs are more expensive than clubs purchased “off the shelf” from your local golf supplier or even from an outlet store or individual, the results will probably show themselves right away on the golf course.
Consider the shots that you may have been compensating for all your years of playing golf. If you’re taller or shorter than the “typical” golfer, or even if you have some muscle strengths or weaknesses that make your game a bit more of a challenge, custom clubs can help you compensate.
But what if you want the clubs immediately? Do you have to go with “off the shelf” clubs? Actually, many manufacturers who provide custom golf clubs promise delivery within just a few days, a week or two at most.
Among the few negatives of custom golf clubs is the potential for offering them up as a surprise gift and the potential for resale. But at the same time, the golfer who is going to receive the custom clubs will likely gladly give up the “surprise” and most who go to this expense and trouble aren’t going to offer the clubs for sale.
Only you can decide whether custom golf clubs are a good investment for you. If you’re going to spend a bundle of money for an awesome set of clubs, you may very well take the extra step to get custom clubs.

Use an automatic LED night light so it’s easy to find and turn on the bedside lamp when getting up at night.
Place a sturdy chair in the bedroom so your older adult can sit while dressing to prevent falls.
Remove any throw rugs, electrical wires, or clutter from the floor to prevent tripping and falling.
Clean and organize closets and drawers so frequently used items are within easy reach — avoid storing anything too high or too low.
Make sure the bed is at the correct height so it’s easy to get in and out. Place the box spring on the floor if it’s too high or use inexpensive bed lifts if it’s too low.
Install an easy-to-use phone by the bed and post important phone numbers in large print.
Make sure a smoke detector is installed on or near the ceiling. Get one with 10 year battery and low false alarm rate for low maintenance.

I can’t stand the idea of exercising every day for at least 30 minutes.
That’s okay because exercising for several 10-minute periods a day can be just as effective as one 30-minute period. To get some ten-minute exercises, park in the farthest space at the shopping mall, or leave the car a couple of blocks away from your destination. Take the stairs instead of the elevator. Do some of your own housekeeping or gardening.
I’ve always heard “no pain, no gain” and I don’t want to be in pain.
When you exercise, you should raise your heartbeat and breathe a little heavily, but you shouldn’t be hurting. In fact, if you push yourself so hard that you hurt, you may injure yourself. You can get plenty of good results when you exercise without pain. In fact, if you exercise in short ten-minute spurts, you may not even perspire heavily!
I’m already tired. Exercising will just make me more exhausted.
Physical activity will actually make you more alert – and will help you sleep better at night, which will make you less tired during the day. When you exercise, endorphins are released in your body. These make you more relaxed and give you energy!
I’m not sure what exercise programs are best for me.
As a senior, you may have specific conditions or diseases that should be taken into account before you begin an exercise program. Talk to your doctor about what type of exercise is best for you , and what types of exercise you should avoid. Your doctor may be able to connect you with a physical therapist who can assess your strengths and weaknesses and advise you about an exercise plan. A fitness instructor at the gym can do the same, but make sure you tell him or her about your limitations.
I can’t exercise because of a medical condition or physical limitation.
If you have a disease or disability, you may not be able to exercise as you once did – and this can make you feel frustrated or depressed. Don’t stop exercising now! This is when you need physical activity the most. Exercise can make you feel much better physically, mentally and emotionally – and can improve your mobility and reduce frailty.
Instead of focusing on what you can’t do, think about what you can do. Positive thoughts produce positive results. Talk to your doctor to find out what type of physical activity is best for you, and ask your doctor to help you locate a health professional to help design your exercise plan.
I don’t have the money to join a gym or buy expensive equipment.
There are lots of ways to exercise without ever walking in the door of a gym or purchasing fitness equipment! You can do lots of activities at home – indoors or out, with no specialized equipment. Meeting a friend for a walk or bike ride. Take a dance class at the senior center. Use resistance bands against your furniture, floors and walls to increase your strength. None of these activities require you to join a gym or buy equipment.
I don’t know how to use the equipment at my gym.
Are you avoiding the gym because you don’t understand the equipment? Make an appointment with an instructor at your gym who can teach you how to use the equipment the right way. If you haven’t already joined a gym, talk to other seniors and your doctor to find out which gyms connect well with seniors and are less intimidating. The local YMCA and senior center are good places to start. NOTE: Always talk with your doctor before beginning an exercise program. Every type of exercise is not appropriate for all people, especially if you have high blood pressure, heart disease or diabetes. And it’s important that you start slowly with any physical activity.
